| *A $100 per session deposit must accompany the application or it will not be processed. A full payment is due by June 1. |
Health Form (Physical & Immunizations)

Every camper must submit the required Health Form in order to participate in any Stonehill College Sports Camp. If the camper chooses to register online, their confirmation email will include the required Health Form, Release of Liability and Medication Release (if camper is planning to take medication at any time during camp) forms. The Health Form can be downloaded by clicking the link above.

Release of Liability

Every camper must submit the required Release of Liability in order to participate in any Stonehill College Sports Camp. Medication Release Form

If the camper will be taking medication at any time during camp they must submit a Medication Release Form. If the camper chooses to register online, their confirmation email will include the required Health Form, Release of Liability and Medication Release (if camper is planning to take medication at any time during camp) forms. The Medication Release Form can be downloaded by clicking the link above.

Each Stonehill College Sports Camp must comply with regulations of the Massachusetts Department of Public Health and be licensed by the local Board of Health (Easton).
