2010 Health Form (Physical & Immunizations)

Every camper must submit the required Health Form in order to participate in any Stonehill College Sports Camp. If the camper chooses to register online, their confirmation email will include the required Health Form, Release of Liability and Medication Release (if camper is planning to take medication at any time during camp) forms. The Health Form can be downloaded by clicking the link above. Each form must be submitted by sending them to the following address: |

If the camper chooses to register online, their confirmation email will include the required Health Form, Release of Liability and Medication Release (if camper is planning to take medication at any time during camp) forms. Each Stonehill College Sports Camp must comply with regulations of the Massachusetts Department of Public Health and be licensed by the local Board of Health (Easton).
